In order for us to know who you are and what you like, you will need to complete some forms. These will tell us more about you and assist us in making your time in Streetline a happy one.
Below we have added some forms that you can complete. We also added some information about fostering and residential care which you should look at to understand your current and future care situations.

Care Plan Form: This form is to tell us more about you, what you like and your background. Please open this PDF and complete it carefully, print and hand it back to us.
Residential Guide: This guide will give you more information about your care. This is a PDF document that can be printed off for your convenience.
Foster Care: The government has a set of standards about living in foster care which is written to make sure you are kept safe and cared for properly. This is a PDF that can be printed to keep.
Residential care: There are also a set of standards set up by the government to ensure your residential care will protect and respect you.
All of the above are there to assist us and guide you in your fostering and residential care. Once we know you better (and you know what you can expect) we can hopefully start a long and happy road together.
