Not Happy…

These forms will help us to better understand what is working and what is not working in Streetline (grievance). It also gives you the opportunity to let management and social workers know if something has happened that you feel is wrong and we need to know about (complaint).

These forms are for SERIOUS COMPLAINTS or GRIEVANCES and will be emailed to the manager.

Below we have two forms, both are the same. Please make sure you choose the correct one.

The form below is for all people currently living or residing at Streetline.

STREETLINE RESIDENCE COMPLAINT

The form below is for Streetline staff, ex-residence or other members of the public.

NON-RESIDENCE COMPLAINT

 

Information for Young People on the Complaints Procedure:

It is important that all young people at Streetline feel safe. If a young person is unhappy about something or they feel they are being treated unfairly, then the young person can make a formal complaint.

How to make a complaint:

Inform any member of staff at Streetline that you wish to make a complaint.The staff member will record your complaint in the complaints book which you can sign too.

What happens next:

The manager of Streetline will be told about your complaint, as will your parents/guardian and social worker and anybody else that needs to know.

Your complaint will be dealt with in three days, if can’t be dealt with in three days, then you will be give a reason why it cannot be done in this time.

The Streetline manager will meet with you and go through your complaint to make sure that she understands your complaint and has enough information.

When the manager finishes her investigation she will meet with you again and tell you the outcome of the investigation.

What can you do if you’re not happy with the outcome of your complaint?

If you are not happy with the outcome you can appeal. What this means is that some else like your social worker or another professional not involved in your complaint will investigate your complaint.

Must a complaint be made to staff of Streetline?

Your complaint can be made to someone outside of Streetline such as your social worker, relative or a guardian can raise the complaint on your behalf.
